Mentor Application

Become a MOSTe mentor and help change a girl’s life! To apply, please download and fill out these forms:

1. Mentor Description
2. Mentor Application
3. Live Scan Form  – Please complete this form, take it to a service provider near you and get fingerprinted (this should cost no more than $20-$25).  The results will be sent directly to MOSTe by the service provider. Find a nearby location HERE.
4. Driver Record Request – Please complete this request and send the results to MOSTe. If you would like to be reimbursed for the $2 fee, please include a copy of your receipt.
